Screening and Consent form

 

Dear Parent or Guardian, for most juniors , physical activity is very beneficial, but there are a small number who may be at risk when participating in an exercise program. Your child will not be able to participate in any exercise program until this form has been completed and reviewed by a trainer.  Please note that all information is confidential.

Specially designed teens programmes are available in the gym to use and trainers are on hand to help.

Young Adults can attend unaccompanied, but a full induction is required along with parental consent.

For a small charge we often run 4 week small group strength courses aimed to support good movement patterns and weight lifting technique. This is for members and non members - so friends can attend. Do look out for the next available courses.

The information collected in this form will be used only for the purpose of designing an appropriate fitness programme/ workout for you and will not be disclosed to any third party without your consent.

We reserve the right to request a doctor’s clearance before allowing you to receive training with Kate Campbell Fitness.

If your health condition changes you must inform the Kate Campbell fitness team and if necessary complete another screening form.

If you have any injuries or known medical conditions please inform your instructor before each session and class to enable modifications and adaptations to be made
